Waiver and Release of Liability Agreement

By selecting “I Agree” and/or electronically signing below, I acknowledge that I have read, understood, and agree to be legally bound by the terms of this agreement.

1. Assumption of Risk

I understand that participation in dance classes, rehearsals, events, and other physical activities at Studio Kamel Dance Studio (“Studio”) involves inherent risks, including but not limited to physical injury, muscle strain, slips, falls, and other potential hazards. I voluntarily assume all such risks.

2. Release of Liability

To the fullest extent permitted by law, I hereby release, waive, and discharge Studio Kamel Dance Studio, its owner(s), instructors, employees, contractors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, damage, or loss that may occur as a result of my participation.

3. Medical Responsibility

I confirm that I am physically able to participate in dance and fitness activities. I understand that the Studio does not provide medical advice or supervision. I agree to stop participation immediately if I feel pain, discomfort, or any health concern.

4. Personal Responsibility

I agree to follow all Studio rules, instructions, and safety guidelines. I understand that failure to do so may increase my risk of injury.

5. Indemnification

I agree to indemnify and hold harmless Studio Kamel Dance Studio and its staff from any claims brought by third parties arising from my actions or participation.

6. Photo/Video Disclaimer

I understand that photos or videos may be taken during classes or events. I acknowledge that a separate media release governs usage of such materials.

7. Governing Law

This agreement shall be governed by and interpreted in accordance with the laws of the Commonwealth of Virginia.

8. Electronic Signature Consent

I agree that selecting “I Agree,” checking a box, typing my name, or otherwise electronically signing this document constitutes my legal signature and is equivalent to a handwritten signature under applicable law, including the U.S. Electronic Signatures in Global and National Commerce Act (E-SIGN Act).

By signing below, I confirm that:

  • I have read this agreement in full
  • I understand its contents
  • I voluntarily agree to all terms
